(see list on back) PROTECT SITE OF SOIL ANALYSIS!

2. Call and schedule a pre-construction site evaluation.

3. Complete and return an application. One half of the permit fee will be due when returning the application.

4. A specification sheet will be sent to you. This is to be completed by a licensed installer.

5. Select a licensed installer from the list on the reverse side, to complete the specification sheet with their design plan. This design plan must be submitted for review and approval. Upon approval and the payment of the remaining balance, a permit will be issued.

6. The Knox County Area Plan office will be notified of this approval. The permit shall lapse and be void if work has not been completed, including inspection within 1 year after its issuance.
